Short-term Rehabilitation

Short-term Rehab is available to our residents and is a critical part of recovering and developing independence after a stay in an acute hospital. The Tamalpais supports our residents in need of rehabilitation so they can get back to living independently as soon as possible. Short-term Rehab is also available to community residents. We also have Short-term Rehab at The Sequoias San Francisco and The Sequoias Portola Valley

At The Tamalpais, we offer comprehensive medical rehabilitation from our on-site physicians, licensed nurses and physical, occupational, and speech therapists. We have highly trained, experienced staff and the facilities needed to enable our patients to reach their highest level of recovery. We also have the same type of excellent care at The Sequoias San Francisco and The Sequoias Portola Valley.

Criteria/Guidelines

To qualify for temporary residency and rehabilitation you must:

  • Be insured through traditional Medicare A, or a contracted HMO/PPO/Medicare Advantage plan
  • Have a three-day qualifying stay in an acute hospital
  • Require daily skilled nursing services or oversight from a licensed nurse or therapist
  • Be able to participate in treatment and make measurable progress during the course of care
  • Have a solid discharge plan and arrangements for post-treatment care

Transitional Care

We know the importance of communication between our team and your physician. This process, known as the transitional care model, can help make the rehabilitation process successful.

Accommodations and Amenities

We do everything we can to make a patient’s stay as comfortable as possible. We offer private and semi-private rooms, social dining, activities, assistance, monitoring and more.

Each therapy program is customized to meet an individual patient’s goals following an illness, injury or surgery.

Our rehabilitation fitness areas offer state-of-the-art equipment and treatments to help a patient’s recovery. We also offer a supervised fitness center for general exercise.
